How to Register

(Registration Begins April 13, 2010)

 

1. Pick up a registration form at SWCC or here to download form.

 

2. Registration form with payment for each  

    child due on June 14, 2010.

    (first come first served)

 

3. The $5.00 Annual Drop-In Program fee is  

    also due on June 14, 2010.

 

4. Mail registration form and fee to:

 

Susannah Wesley Community Center

               Attn: Jann Cabacungan

                    1117 Kaili Street

              Honolulu, HI 96819-3432

 

    Walk-in registration accepted Monday    

    through Friday from 9 a.m. to 4 p.m.

 

5. Fees may be paid by cash or check/money

    order payable to:

    Susannah Wesley Community Center    

    ($15.00 fee for returned checks)

 

6. There is no telephone registration.
