Care Coordination

Over 100 years ago, SWCC was established to improve the quality of life for newly arrived immigrants in Hawaii. We continue that proud tradition today through our Behavioral Health Care Coordination Services for adults struggling with mental illness.

Our Care Coordination includes:

• Outreach
• Screening
• Assessment
• Supportive counseling
• Crisis intervention
• Education and Advocacy
• Family Support
• Referral and Linkage
• Monitoring and Follow-up
• Interpretation and Translation

We currently provide Care Coordination in the following languages:

• Filipino (Ilokano & Tagalog)
• Korean
• Samoan
• Vietnamese
• Chinese (Cantonese & Mandarin)
• Chuukese
• Japanese
• English

Currently, the program receives referrals through a contract with the Department of Health Adult Mental Health Division and APS Healthcare – Community Care Services. We provide assistance to access these programs and support persons seeking help through the eligibility determination process.
